Community Sports and Therapy Center is located in Mercer County Community Hospital, 800 West Main Street, in Coldwater, Ohio and at 1107 North Main Street, Suite 8 in Celina, Ohio.

Programs are also available at the Auglaize-Mercer Family YMCA locations on State Route 703 between Celina and St. Marys, Ohio and on Wuebker Road between New Bremen and Minster, Ohio. For those patients unable to come into our outpatient centers, we can provide Therapy Services in your home through Mercer Health Home Care.
